Biochemistry
Your liver does dozens of jobs at once — filtering toxins, making proteins your blood needs, producing bile for digestion — and a Liver Function Test is really a panel of several smaller tests, each checking a different one of those jobs from a different angle. Enzymes like ALT and AST leak into your bloodstream when liver cells are damaged, so raised levels are usually the first sign something's wrong. Other parts of the panel, like albumin and bilirubin, measure whether the liver is still doing its normal work properly, not just whether cells are being injured. Reading the whole panel together, rather than any single number, is what actually tells a doctor which kind of liver problem might be present.
ALT and AST are enzymes that rise when liver cells are damaged or inflamed — ALT is considered more liver-specific, while AST also comes from muscle and the heart. Alkaline phosphatase rises more specifically when bile flow is blocked, pointing toward a different kind of liver problem than a raised ALT would. Bilirubin, the pigment that turns skin and eyes yellow in jaundice, shows whether the liver is properly processing and clearing waste. Albumin, a protein made only by the liver, reflects its longer-term, ongoing function rather than a sudden injury.
The pattern across the panel matters more than any single result, and several of its parts are only meaningful read together. Alkaline phosphatase (ALP) is a good example: it's found throughout the body, with higher amounts specifically in the liver, bile ducts, and bones, so a raised reading on its own doesn't say which of those is involved. That's exactly where gamma-glutamyl transferase (GGT) — another enzyme, sometimes included in the same panel — earns its place: both GGT and ALP tend to rise together in liver and bile-duct problems, but bone conditions raise ALP alone, leaving GGT normal or low. Seeing the two together is what actually points to which organ is involved, not either number by itself.
Bilirubin and albumin round out the panel by measuring different jobs entirely. Bilirubin is the waste pigment behind jaundice, so its level shows how well the liver is clearing it. Albumin is a protein the liver alone produces, so a low reading — especially alongside other abnormal results — can suggest its longer-term, ongoing function is affected, rather than a sudden injury.
A single abnormal liver test is common and often temporary — recent alcohol, certain painkillers, or even strenuous exercise can raise ALT or AST briefly. That's why doctors often repeat an abnormal panel before concluding anything, rather than acting on one result alone — a raised number today doesn't automatically mean an ongoing problem tomorrow. The test itself is a routine blood draw from a vein, with no special preparation of its own beyond whatever any other tests on the same form might separately require.
It leans toward liver-specific injury, since ALT is found mostly in the liver while AST also comes from muscle and the heart. Your doctor will look at the full pattern, not just this one difference, to interpret it.
Not necessarily. A single raised enzyme is common and can come from something temporary — recent alcohol, certain medicines, even hard exercise — which is why doctors often repeat the test before drawing conclusions.
Bilirubin is a waste product the liver normally clears from your blood. When it builds up instead, it shows up as jaundice — yellowing of the skin and eyes — so measuring it directly tells your doctor whether that clearing process is working.
This page is general information about the test, not medical advice. Your own report's reference ranges apply — please discuss your results with your doctor.
It depends on what else is ordered alongside it — the liver panel itself doesn't strictly require fasting, but it's often combined with other tests, like a lipid profile, that do. Check the full list on your form.
ALT and AST rise mainly when liver cells themselves are damaged. Alkaline phosphatase rises more specifically when bile flow out of the liver is blocked — a different kind of problem, even though both show up on the same panel.
Yes, strenuous exercise shortly before the test can temporarily raise AST, since this enzyme is also released from muscle tissue, not just the liver.
Several common medicines can affect the liver, so a baseline reading lets your doctor compare later results and catch a problem early if one develops.
High enzymes usually signal active injury or inflammation happening now. Low albumin instead reflects the liver's longer-term ability to make proteins, so it points toward a more chronic issue rather than a sudden one.
